NutriSimple is a private company from Quebec. More specifically, NutriSimple offers clinical nutrition services to a diversified client based and offers personalized consultation services in nutrition for their clients.

NutriSimple recognizes and respects the privacy of individuals regarding the collection, processing and storage of personal information.

INTENT AND SCOPE OF THIS POLICY

This Privacy Policy covers how NutriSimple collects, uses and discloses nutritionists’ and clients’ personal information as well as how their personal information can be made available and corrected if need be. This policy explains the use of cookies for users of NutriSimple’s website.

By accessing NutriSimple’s website, at nutrisimple.com, contacting or agreeing to receive information by e-mail from NutriSimple, users agree to the terms and conditions of this policy.

This policy does not cover websites operated by third parties. Therefore, NutriSimple is not responsible for the privacy policies, procedures and practices of these websites.

COLLECTION OF PERSONAL INFORMATION

1. Information provided by nutritionists and clients

NutriSimple collects personal information in various ways, particularly when users: 

-        In general:

• Contact the company by phone or by e-mail;
• Contact the company through the contact page of nutrisimple.com;
• Post on their social media accounts; 
• Purchase products or our services via the online store of NutriSimple;
• Create an account on NutriSimple’s website, at nutrisimple.com;
• Subscribe to the newsletter.

-        More specifically, when an individual client:

• Fills out the health questionnaire to prepare for a visit.

Depending on the circumstances, this may include:

-        As a client:

• First name(s) and last name(s);
• Postal details, phone numbers and electronic contact details;
• The nature of the appointment (for example: weight loss, celiac disease, diabetes, etc.); 
• Lifestyle habits (for example: sedentary or not; type of food)

2. Information provided through our websites

As most of other commercial websites, NutriSimple uses cookies. Cookies are files installed on your hard drive or on your web browser to obtain information such as language preference, browsing history and the browser version your computer uses, to further improve nutrisimple.com during your visits.

These cookies do not collect any personal information.

Deactivating cookies may impair the navigation experience on nutrisimple.com.

USE AND DISCLOSURE OF YOUR PERSONAL INFORMATION

NutriSimple mainly uses and discloses personal information:

• To offer products and services, particularly booking appointments and providing nutrition services;
• To answer questions and information requests;
• To comply with the legal obligations and book-keeping requirements;
• To protect itself against fraud;
• For statistical purposes to improve its services.

Access to personal information will only be granted to employees who needs access to personal information of clients to fulfill their job requirements. All employees signed a non-disclosure agreement for personal information. Nutritionist-dietitians are members of the Ordre professionnel des diététistes du Québec and have the obligations to retain clients’ records for at least 5 years after the last consultation.

RIGHT OF ACCESS AND RIGHT OF RECTIFICATION

Upon written request, and proof of identification, users can have the right to get access to their personal information collected, used and disclosed by NutriSimple, and request correction of information, if need be, required or authorised by law.

However, to make sure that all personal information we hold are accurate and up to date, NutriSimple invites users to inform them without delay about any change of their personal information.

UPDATE

NutriSimple reserves the right to amend this Privacy Policy from time to time. NutriSimple Privacy Policy was updated on April 4, 2019.
